A Personal Care Directive is a document you execute to appoint a person to make your health and personal care decisions when you are no longer able to do so.

You may also, although you are not required, provide specific instructions to the person you appoint as to your wishes with respect to certain health related matters. For example, you may indicate in the document that you do not wish to be provided certain medical treatments in certain circumstances. This document is what is often colloquially referred to as a personal care directive or a “living will”.
